ABSTRACT:

Practice related to demonstration on pulmonary rehabilitation techniques are posing a great threat globally in COPD patient's mortality and morbidity, COPD is an increasing health problem globally and India is not an exemption. The patients have poor practice related to pulmonary rehabilitation techniques. Present study is aimed to assess the effectiveness of demonstration on practice regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease in selected hospital at Udaipur city, Rajasthan. 60 samples was selected using Non probability purposive sampling techniques. The findings revealed that In pre test 88.33% respondents were found with poor level of practice whereas with post test 21.67% of the respondents had poor practice level, 11.67% respondents had average practice level in the pre test whereas in post test it was 75.00%, there were no respondents found with good practice level in pre test whereas in post test good respondents level improved to 3.33%. The study concluded that there was significant improvement in the level of practice of patient with COPD regarding pulmonary rehabilitation techniques. Which indicated that the demonstration was effective. The study also revealed that there is a significant association between pre test practice scores with selected socio demographic variables. Hence this kind of practice should be conducted from time to time for patients so that their practice can be improved by pulmonary rehabilitation techniques and reduce the breathing complications in COPD patients.

INTRODUCTION:

Human respiratory system not only provides oxygen to each cell of the body but also removes body wastes, filters out infectious agents, and provides air needed for speech. Although the lungs are able to with stand abuse in the form of smoke and other pollutants, a number of disorders impair its function.1


COPD is a polygenic disease as it results from geneenvironment interaction. Like all other chronic diseases, COPD has modifiable and non-modifiable risk factors that are preventable. Cigarette smoking is the commonest risk factor noticed globally, but various epidemiological studies have got enough evidence that non-smokers may also develop COPD.2 Pulmonary rehabilitation does not attempt to change or improve  lung function,   improvements   in   exercise   tolerance, quality of life and breathlessness are achieved through mechanisms other than change in pulmonary function, that is increasing muscle strength, achieving desensitization to breathlessness and improving aerobic fitness. Impairments in lung function and ability can be addressed through treatments such as exercises so the patient's respiratory function can be restored to the maximum possible to increase the patient's independence and overall quality of life. 3

 

NEED FOR STUDY:

Chronic Obstructive Pulmonary Disease was the second highest cause of death in India after heart disease in 2017, killing 1 million (958,000) Indians that year, according to University of Washington’s Global Burden of Disease study 2018.4 Chronic Obstructive Pulmonary Disease (COPD) result of increasing airway resistance secondary to bronchial mucosal oedema or smooth muscle contraction. It affects the movements of air in and out of lungs. It includes chronic bronchitis and emphysema.5 Studies conducted concluded that the demonstration significantly increased the practice of patients with COPD regarding pulmonary rehabilitation techniques. The demonstration was effective to enhance the practice of patients so these kind of strategies can be used in the hospitals, community to increase practice regarding pulmonary rehabilitation techniques and to reduce morbidity and mortality rate.

 

PROBLEM STATEMENT:

Effectiveness of demonstration on practice regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease in selected hospital at Udaipur city, Rajasthan.

 

OBJECTIVES:

1.               To assess the pre test practice scores regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease.

2.               To assess the post test practice scores regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease.

3.               To evaluate the effectiveness of demonstration on practice regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease.

4.               To find out association between pre test practice scores of patients regarding pulmonary rehabilitation techniques with selected socio demographic variables.

 

Hypothesis:

H1                           There is a significant difference between mean pre test and post test practice scores regarding pulmonary rehabilitation techniques among patients with chronic obstructive pulmonary disease.


H2                     There is a significant association between mean pre test practice scores and selected socio demographic variables.

 

METHODOLOGY:

Research approach and design:

To assess the effectiveness of demonstration on practice regarding pulmonary rehabilitation techniques among patients with COPD a pre experimental research design with evaluative approach was used.

 

Setting of the study:

The study was undertaken in medical wards Maharana bhupal government hospital Udaipur

 

Population:

The population consisted of patients in medical wards of Maharana bhupal government hospital Udaipur (Raj.).

 

Samples:

Samples of the study were patients with COPD in medical wards of Maharana bhupal government hospital Udaipur City, Rajasthan.

 

Sample size:

Sample size was 60 patients with COPD.

 

Sampling technique:

In this study  the samples were selected through Non probability purposive sampling techniques.

 

Inclusion Criteria:

Patients who were

·           Admitted with COPD

·           Willing to participate in the study.

 

Exclusion Criteria:

Patients who were

·           Not consented to participate in study.

·           Having co-morbidties.

 

Description of the Tool:

The tool consisted of two sections

 

Section A:

Described the selected sample characteristics. It comprised of 7 items for obtaining information regarding age in years, education, gender, monthly income, occupation, previous knowledge regarding pulmonary rehabilitation techniques, source of information.

 

Section B:

Consisted of checklist on pulmonary rehabilitation techniques consisted of breathing exercise, use of inhaler, postural drainage.


Validity of the Tool:

Items were judged by experts for relevance, clarity and appropriateness. Modifications done as per expert opinion.

 

Reliability of the Tool:

The reliability of co-efficient of internal consistency was computed by using Cronbach Alpha.

 


Data Collection Procedure:

Written permission was obtained from ethical committee of RNT medical college, Udaipur prior to data collection. The sample were patients with COPD admitted in medical wards at Maharana bhupal government hospital , Udaipur. The purpose of the study was explained and obtained informed consent from patients.

 

Plan for data analysis:

Data was analyzed as follows:

A master data sheet was prepared by the investigator. Distribution of sample according to socio-demographic variables was done by using frequency and percentage distribution.

 

Mean, mean % median and standard deviation used to analysed pre test and post test practice scores.

 

Anova test was used to find out the association between practice scores and socio-demographic variables of patients with COPD in selected hospital at Udaipur city.

 

RESULTS:

The demographic characteristics of the respondents revealed that

·                 Majority (38.33%) of participants was from the age group of 61-70 years.

·                 Majority of the respondents (70.00%) were male.

·                 Distribution of respondents according to educational qualification revealed that majority (41.67%) were Illiterate.

·                 Majority (35.00%) of the participants had a monthly income below 5000.

·                 Distribution of respondents according to Occupation revealed that (35%) were farmer.

·                 Majority of the participants 60 were not having previous knowledge regarding pulmonary rehabilitation techniques.

Association between Pre test Practice Scores with Selected Socio-Demographic Variables.

Association between mean pre test practice scores and selected socio demographic variables such as Education and Monthly Income. But there was no significant association between mean pre test practice scores and selected socio demographic variables such as age, gender, occupation.

 

RECOMMENDATIONS:

·                 Similar study can be replicated on a larger sample and their findings can be generalized for a larger population.

·                 An comparative study can be conducted in Private and Government hospitals.

·                 More in-depth studies can be conducted on the topic.

·                 Demonstration on Pulmonary rehabilitation techniques among patients with COPD.
