Knowledge and Attitude of Care Givers regarding Home Care Management of Stroke Patients

ABSTRACT:

Stroke, also known as cerebrovascular accident (CVA) or brain attack is a vascular disease caused by an interruption in the blood supply to the parts of brain.

Aims and objectives: The study aim is to assess the knowledge and attitude of caregivers regarding home care management of stroke patients.

Approach and design: The descriptive study approach was used.

Sample and Sampling criteria: Non probability convenience sampling technique was adopted to select 60 care givers of stroke patients in J.S.S. Hospital at Mysuru.

Tools and techniques: Structured interview schedule was used to collect the data. The data were collected and analysed using descriptive and inferential statistics.

Result: Result revealed that majority 25(41.65%) care givers have average knowledge, 23(38.3%) have good knowledge and 12(20%) care givers have poor knowledge. Majority 40(66.6%) care givers have unfavourable attitude and 20 (33.3%) have favourable attitude.

Conclusion: It was concluded that knowledge is the basis for development of positive attitude. There should be more provision to increase the awareness of caregivers of stroke patients regarding prevention, home care management and rehabilitation of stroke.

INTRODUCTION:

Stroke or cerebrovascular accident is defined as the sudden or acute focal neurological deficit, non-convulsive in nature that results from pathologic process involving cerebral blood vessels. About 7, 95,000 Americans each year suffer a new or recurrent stroke. About 40% of stroke deaths occur in males and 60% in females. In 2010, worldwide prevalence of stroke was 33 million, with 16.9 million people having a first stroke.

 

World Health Organisation estimates suggest that by 2050, 80% stroke cases in the world would occur in low and middle income countries mainly India and China. Indian council of medical research estimated that in 2004, there were 9.3 lakh cases of stroke and 6.4 lakh deaths due to stroke in India, most of the people being less than 45 years of age. The high incidence and high prevalence of stroke have a major impact on society. After initial hospitalization and stroke rehabilitation, 80% of stroke survivors return to the community, relying on their family members’ emotional, informational, and instrumental support for daily living. Stroke caregivers have to deal with not only stroke patients’ difficulties in mobility, self-care, and communication, but also their cognitive impairment, depression, and personality changes.

STATEMENT OF THE PROBLEM:

“A study to assess the knowledge and attitude of care givers regarding home care management of stroke patients in JSS hospital at Mysuru with a view to develop health information pamphlet”

 

OBJECTIVES OF THE STUDY:

1.        To assess the level of knowledge and attitude of care givers regarding home care management of stroke patients in JSS hospital at Mysuru.

2.        To find the relationship between the level of knowledge and attitude of care givers on home care management of stroke patients.

3.        To find the association between the level of knowledge and attitude of care givers regarding home care management of stroke patients with their selected personal variables.

 

HYPOTHESES

The following hypotheses are formulated for the study and will be tested at 0.05 level of significance:

H1:There will be significant correlation between level of knowledge and attitude of care givers regarding home care management of stroke patients.

H2:There will be a significant association between the level of knowledge of care givers regarding home care management of stroke patients with their personal variables.

H3:There will be significant association between level of attitude of care givers regarding home care management of stroke patients with their personal variables.

 

METHODOLOGY:

Setting of the study:

JSS hospital, Mysuru, with 1800 bedded hospital.                                                    

 

Sample and sample size:

Care givers of 60 stroke patients in JSS hospital, Mysuru.

 

Sampling technique:

Non-probability convenience sampling technique.

 

Criteria for sampling:

Inclusion criteria:

Care givers who are,

·         Willing to participate in the study.

·         Who can understand Kannada language.

·         Available during the study.

·         Both the genders irrespective of relationship.

 

Exclusion criteria:

Care givers who do not take care of stroke patients in JSS hospital.

 

Tools and instruments used

1.        Proforma of selected personal variables.

2.        Structured interview schedule to assess the knowledge of care givers regarding home care management of stroke patients in JSS hospital, Mysusru.

3.        Attitude scale to assess the attitude among care givers regarding home care management of stroke patients in JSS hospital, Mysuru.

Data analysis by Descriptive and Inferential Statistics.

 

RESULTS:

The results are presented under following sections:

1.        Findings related to selected personal variables of care givers regarding home care management of stroke patients.

2.        Findings related to knowledge and attitude of care givers regarding home care management of stroke patients

3.        Findings related to relationship between knowledge and attitude of care givers regarding home care management of stroke patients.

4.        Findings related to association between level of knowledge and attitude of care givers and their selected personal variables.

 

1.       Findings related to selected personal variables of care givers regarding home care management of stroke patients.

Findings of the present study that majority 17(28.3%) caregivers were in the age group 40-50 years. majority 33(55%) care givers were female. Most 34(56.6%) of the caregiver belongs to nuclear family.  Majority 22(36.6%) had secondary education, majority 31(51.6%) do agriculture. 49(81.6%) care givers belongs to hindu religion, majority 22(36.6%) were having  monthly income of the family of Rs 6000- 9000,  most 17(28.3%) of the care givers had experience in caring stroke patient from 6 months -1 year, majority 20(33.3%) caregivers were wife’s.   

 

2.       Findings related to knowledge and attitude of care givers regarding home care management of stroke patients.

Findings showed that, majority 25(41.6%) caregivers had average knowledge, 23(38.3%) had good knowledge and 12(20%) poor knowledge regarding home care management of stroke patients and the knowledge score of caregivers regarding home care management of stroke patients ranged from 9-25, with the mean knowledge score 17.5 with standard deviation of ±7.4. the knowledge of caregivers regarding home care management of stroke patients was satisfactory.

 

The attitude scores of care givers showed that majority 40(66.6%) of care givers had unfavourable attitude and 20(33.3%) had favourable attitude regarding home care management of stroke patients and the attitude scores ranged from 64-82, with the mean attitude score 72 and standard deviation of ±4.3.

 

The above findings stress the need for educational and awareness programmes regarding home care management of stroke patients.

 

3.       Findings related to relationship between knowledge and attitude of care givers regarding home care management of stroke patients.

To find the relationship between knowledge and attitude of care givers regarding home care management of stroke patients correlation of co-efficient was computed and obtained value is r=0.7 it is significant at 0.05 level. Thus, the research hypothesis is H1 was accepted inferring that there is significant relationship between knowledge and attitude scores of care givers regarding home care management of stroke patients. As the knowledge increased attitude also became more favourable and vice versa.

 

4.       Findings related to association between level of knowledge and attitude of care givers and their selected personal variables.

There is significant association found with knowledge scores of care givers regarding home care management of stroke patients and their selected personal variables like type of family, education, occupation, monthly income, experience and relationship with the patient. With regard to the association between level of attitude and their selected personal variables, there is significant association between attitude and their selected personal variables like type of family and experience.

 

CONCLUSION:

It is evident from the statistical analysis that care givers of stroke patient have average knowledge than good knowledge and unfavourable attitude than favourable attitude regarding home care management of stroke patients, it suggests that there is need for improving knowledge and attitude of caregivers in home care management of stroke patient.
