ABSTRACT:

The reduction of pain among post caesarean section mothers has an important role to play in enabling effectiveness of Benson’s relaxation therapy intervention as an independent nursing intervention. The objective of the study is to assess the effectiveness of Benson’s relaxation therapy on reduction of pain among post caesarean section mother. The research design adopted was quasi-experimental time series design. The conceptual framework for this study was based on General system theory modified Ludwig Von Bertalanffy’s. The study had been conducted in Shree Mehta Sarvajanic Hospital At Una, Girsomnath District. Non-probability purposive sampling technique had been adopted to select the desired samples. The sample size was 60. As an intervention Benson’s relaxation therapy was administered for 1st to 3rd post operative day for 5 minute twice a daily in the morning and evening, and then gradually the time is increase 10 - 20 minutes. The collected data were analysed by using both descriptive and inferential statistical methods,’ F’ value was used to assess the effectiveness of Benson’s relaxation therapy on reduction of pain. The obtained experimental group ‘F’ value was 34.5*** which showed highly significant at 0.001 level compare to control group ‘F’ value was 14.31 Hence there finding of the study revealed that Benson’s relaxation therapy helps in decreasing the level of pain among post caesarean section mothers with selected demographic variables.

INTRODUCTION:

The pain after Caesarean section  are caused by tissue incision resulting discontinuity of tissue, the stimulation of nerve endings by chemicals released at the time of surgery and the presence of ischemic tissue due to interruption of blood flow to one part of the tissue.

According to research found that 75% of surgical patients experience moderate to severe pain after surgery. Postoperative pain can complicate and delay patient’s recovery and interfere with a patient’s return to activities of daily living.  In many people, pain medications can have unpleasant side effects.1

Recently many methods being developed to address the problem of pain in clients with severe post Caesarean section, either by pharmacological approaches and non-pharmacological. One way non pharmacological suitable to reduce pain intensity Clients post Caesarean section is Benson’s relaxation. Benson’s Relaxation aims to reduce anxiety, decrease muscle tension and bone, as well as indirectly to relieve pain and reduce tension related to the body's physiological. Several studies have shown that the Benson’s relaxation effective in reducing postoperative pain.2

NEED FOR THE STUDY:

Caesarean section is the birth of a foetus accomplished by performing a surgical incision through the maternal abdomen and uterus. It is one of the oldest surgical procedures known throughout history. This alternative option is exercised based on the health status of the mother and child at the time of labor. About 32 percent of mothers prefer planned surgical deliveries. Hence, the outcome of a normal pregnancy can be achieved either through a vaginal delivery or a C-section. The post LSCS discomfort experienced by mothers after cesarean delivery may vary from one woman to another woman.3

A significant rise in institutional and C-section delivery is found between 1992-93 (NFHS-1-national family health survey) and 2007-08 (DLHS-3-District Level Household and Facility Survey) in most of the states in India. The scheme like JSY- Janani Suraksha Yajona may have a great impact on accepting institutional deliveries by poor women. Rising institutional delivery may be a reason of the increase of CS in India. Now recently day by day India Caesarean section rates increased from 25.4 percent to 32 percent. In Gujarat rural area 13.12% and urban area 21.10% has been documented change in annually rate. the proportion of women who have undergone caesarean deliveries is the highest in Kerala (31.8 per cent) and Goa (17.3%) followed by Andhra Pradesh (29.3 per cent) and Tamil Nadu (23.2 per cent) and the lowest in Rajasthan and Jharkhand (4.2 per cent in both the states).4

Now recently people mostly choice a non pharmacological treatment for relieve post operative caesarean section Non pharmacological treatment like a massage, reflexology, yoga, hot application, relaxation therapy etc. Recently most effective therapy is Benson’s relaxation therapy. Benson’s relaxation therapy can help with reduce post operative pain, increase appetite, increase sleep, increase energy, reduce stress, feel relax and indirectly increase bonding with child. Several studies have shown that the Benson’s relaxation effective in reducing post operative pain.5

OBJECTIVE:

·         To assess the level of pain among post caesarean mothers before intervention in both control and experimental group.

·         To assess the effectiveness of Benson’s relaxation therapy on reducing pain among post caesarean mothers in experimental group.

·         To find out the association between post-test level of pain among post caesarean mothers and selected demographical variables in both control and experimental group.

HYPOTHESIS:

1.        H1: There will be statistically significant reduction in the pain level after Benson’s relaxation therapy among post caesarean mothers.

2.     H2: There will be statistically significant association between pain level among post caesarean mothers and selected demographical variables.

MAJOR FINDINGS:

A.      Finding related to demographic variables of the study.

1.        The majority of experimental group 14 (46%) and control group 19 (63%) samples age is between 24-29 years.

2.        The majority of experimental group 18 (60%) and control group 16 (53%) samples are primigravida

3.        The majority of experimental group 20 (67%) and control group 23 (77%) samples are non working.

4.        The majority of experimental group 10 (33.5%) and control group 9 (30%) samples are above education.

5.        The majority of experimental group 11 (37%) and control group 12 (40%) samples weight is between 46-55 kg.

6.        The majority of experimental group 20 (67%) and control group 19 (63%) samples had a emergency caesarean.

B. Findings related to effectiveness of Benson’s relaxation therapy on reduction of pain among post caesarean section mothers.

With regards to effectiveness of Benson’s relaxation therapy on reduction of pain among post caesarean section mothers, the obtained ANOVA test values for the level of pain 34.5 that was very highly significant at p<0.001 level.

C.      Finding related to association between demographic variables and post test.

With regards to the association between the level of pain with their selected demographic variables such as age, parity, education, occupation, weight, nature of caesarean, among this were not significant association found. The shows that the calculated chi-square value is less than tabulated value at the level of 0.05 for all the selected demographic variables, which shows there is nil significant association between effectiveness of Benson’s relaxation therapy and demographic variables.

CONCLUSION:

The main conclusion from this present study is that most of the pain among post caesarean mothers had severe, moderate in pre-test. Moderate, mild and no pain in post-test I, II, III. This shows the imperatives need to understand the purpose of Benson’s relaxation therapy regarding reduction of level of pain among post caesarean mothers and it will improve the quality of life which includes the stability in physiological , psychological, sexual, vocational and lifestyle aspect.
